This app will help you create a Transition Plan.
A Transition Plan is a document that will assist you as you move into adult life. This Transition Plan will update goals and services each year until you graduate.
It was designed for students with Individualized Education Programs (IEPs), but anyone can use it.
A transition plan must be part of the IEP that is in place at the end of your 9th grade year or by the time you turn 16, whichever comes first. It is required by law that your IEP includes transition goals and services. You must be invited to your IEP meeting to discuss this Transition Plan which will assist you as you move into adult life. This transition plan will update goals and services each year until you graduate.
